Payment and Billing

Please note that the pricing information provided is an estimate and Kibun Health cannot guarantee its accuracy. The final amount patients pay is determined by their health insurance coverage. The price list is applicable only to specific services listed and does not include complicating factors or services provided by independent practitioners.

Patient Rights and Protections Against Surprise Medical Bills

It’s important to be aware of your rights and protections against surprise billing or balance billing, particularly when receiving emergency care or treatment from an out-of-network provider at an in-network hospital or ambulatory surgical center. These rights and protections generally apply to individuals enrolled in group health plans, group or individual health insurance coverage, and Federal Employees Health Benefits plans. However, please note that they do not apply to programs such as Medicare, Medicaid, Indiana Health Services, Veterans Affairs Health Care, or TRICARE, as these programs have their own protections against high medical bills.

Balance Billing

Balance billing occurs when you receive care from a health care provider or facility that is not in your health care plan’s network, resulting in additional out-of-pocket costs. However, under certain circumstances, you are protected from balance billing:

  1. Emergency Services:
    • If you have an emergency medical condition and receive emergency services from an out-of-network health care provider or facility, you can only be billed the in-network cost-sharing amount, such as co-payments and co-insurance. Balance billing for emergency services is not permitted unless you provide written consent after reaching a stable condition.
  1. Certain Services at In-Network Facilities:
    • When you receive services from an in-network hospital or ambulatory surgical center, specific health care providers at these facilities may be out-of-network. In such cases, you are only responsible for the in-network cost-sharing amount. This applies to services such as emergency medicine, anesthesia, pathology, radiology, laboratory, neonatology, assistant surgeon, hospitalist, or intensivist services. These health care providers are prohibited from balance billing you unless you provide written consent.

You have the right to retain these balance billing protections and are not obligated to receive care out-of-network. You may choose a provider or facility within your plan’s network.
